DHI hair transplant in Antalya: precise, and possible without shaving

DHI places each graft directly with a fine implanter pen, giving the doctor precise control over angle and depth. It is a favourite for natural hairlines and for patients who would rather not shave their head.

What is a DHI hair transplant?

DHI stands for Direct Hair Implantation. The grafts are extracted from the donor area in the same way as in FUE, one by one with a micro-punch. The difference is how they are placed. Instead of opening all the channels first and then inserting the grafts with forceps, each graft is loaded into a hollow needle inside a Choi implanter pen and implanted directly into the scalp. Creating the channel and placing the graft happen in a single movement.

This gives the doctor very precise control over the angle, direction and depth of every hair, and keeps the time each graft spends outside the body short. It is especially valuable at the hairline, where a few degrees of difference in angle can decide whether a result looks natural or obviously transplanted. Different pen sizes are used for single-hair grafts at the front and thicker grafts behind them, so the density builds up gradually the way a natural hairline does.

Unshaven DHI: a hair transplant nobody notices

One of the main reasons people choose DHI is that it can be performed without shaving the recipient area. Because the pen slides grafts between your existing hairs, you can keep your hair length on top. In many cases only a small strip of the donor area at the back needs to be trimmed, which your remaining hair covers when you comb it down.

Unshaven DHI suits people who cannot take long off work, who have a public-facing job or who simply want the procedure to stay private. It works best for smaller sessions such as hairline refinement or adding density between existing hairs. It is slower and more demanding for the medical team, which is why it usually costs more and why very large graft numbers are often better done with shaving.

Good to know: unshaven DHI hides the procedure well, but the transplanted hairs still shed between weeks three and eight, as with every technique. Your existing hair covers this phase much better than a shaved head would.

Who is DHI best for?

  • Hairline design and refinement, where precise angles matter most.
  • Adding density to thinning areas that still have existing hair.
  • Women with a high hairline or localised thinning who want to avoid shaving.
  • Patients who want to keep their hair long during and after the procedure.
  • Repair work to soften the look of an earlier transplant.

For very large bald areas, FUE or Sapphire FUE is often more efficient, and many clinics combine techniques in one plan: Sapphire FUE to cover the larger area and DHI to build a soft, natural hairline in front of it.

The DHI procedure step by step

  1. Consultation and hairline design, agreeing the exact shape and the graft plan.
  2. Preparing the donor area, trimming only what is needed, followed by local anaesthesia.
  3. Extraction of grafts one by one, kept in a cooled solution.
  4. Loading the pens: a team member loads each graft into a Choi pen of the right size.
  5. Direct implantation of each graft at the planned angle and depth.
  6. Aftercare instructions and medication before you return to your hotel.

A DHI session usually takes a full day. Because the technique is slower, very large sessions may be spread over two days. Your first wash and check-up are usually done at the clinic the next morning.

Recovery after DHI

Recovery after DHI is similar to FUE and often feels slightly lighter, because there are no pre-opened channels. Expect mild tightness and possible swelling for two or three days, small scabs that disappear within seven to ten days and a return to office work after about a week. With unshaven DHI, your existing hair helps to hide the area almost from the start.

Shedding between weeks three and eight is normal, new hair appears at three to four months and the final result is usually visible at around twelve months. Sleeping semi-upright for the first nights, avoiding sport for two weeks and keeping your scalp out of strong sun all help your grafts settle. Our recovery timeline covers every stage.

How much does DHI cost in Antalya?

DHI is usually the most expensive of the three main techniques because it takes longer and needs a larger, more specialised team. In Antalya, all-inclusive DHI packages often start at around €2,200, with unshaven DHI and larger sessions towards the upper end of the €1,800 to €3,500 range and sometimes above. Packages normally include your hotel, VIP transfers, medication and aftercare. Our cost guide explains what to compare between offers.

Frequently asked questions

Is DHI better than FUE?

Neither is better for everyone. DHI offers more control at the hairline and an unshaven option; FUE and Sapphire FUE are more efficient for large areas.

Can I have DHI without shaving?

Often yes, especially for smaller sessions. Usually only a small part of the donor area is trimmed and covered by your remaining hair.

How many grafts can be implanted with DHI?

Many DHI sessions are between 1,500 and 3,000 grafts per day. Larger numbers may be split over two days.

Is DHI painful?

It is done under local anaesthesia. You feel the first injections, then mainly pressure. Mild discomfort afterwards is managed with painkillers.

Why is DHI more expensive?

It is slower, requires more trained staff to load and use the pens, and unshaven work is especially time-consuming.
